Where can I apply for a child health handbook? How can I get a replacement if I lose it?

The Child Health Handbook records the physical health of children at all stages in detail and is a must-have item for babies' birth and growth. So how do you apply for a Child Health Handbook? What is the function of a Child Health Handbook? How do you reissue a Child Health Handbook? The following editor will give you a detailed introduction.

Child Health Handbook

Establishing a "Child Health Handbook" for babies is to carry out systematic health management for children according to the physiological characteristics and health care requirements of children at different stages, promote children's growth and development, improve children's health level, and enable children to develop comprehensively physically and mentally. Children must present a child health handbook to enter kindergartens and institutions. From the time a child is born to the first month of life, parents should go to the maternal and child health care institution where the child is registered or resides to establish a "Child Health Handbook".

How to apply for the Child Health Handbook

The maternal and infant health care handbook shall be established by the community health service center of the street (town) where the pregnant woman is located; when conducting premarital medical examinations or discovering unregistered pregnant women, each premarital medical examination agency and medical institution shall promptly urge the pregnant woman to go to the community health service center where she lives to establish a maternal and infant health care handbook.
Each obstetric medical institution is responsible for establishing a child health handbook before the mother is discharged from the hospital. For children who were not born in the city and do not yet have a child health handbook, the community health service center should promptly establish a handbook for them when they go to the child health clinic for the first time to receive child health services.
The community health service center in each street (town) is responsible for the health check-up of mothers 42 days after delivery, and is responsible for closing the maternal and child health care handbooks. The handbooks will be retained by the community health service center for five years for future reference.
Medical and health care institutions in urban areas that have installed community health service management systems will abolish the original perinatal cards, child health registration cards and numbers, and manage them by numbers in accordance with the city's unified requirements.
The "Maternal and Child Health Handbook" and "Child Health Handbook" are uniformly managed and distributed by maternal and child health centers (stations) in each city (county) and district. The application plan for the following year must be reported to the Basic Maternal and Child Health Department of the Municipal Health Bureau before the end of October each year, and the application must be made in one go.

Child Health Handbook

After establishing the "Child Health Handbook", children should go to the hospital regularly for systematic child health examinations and consultations according to the hospital's agreed time. Examination frequency: 4 examinations for children under 1 year old (3, 6, 9, and 12 months), every 6 months for children aged 1-3 years, and once a year for children aged 3-7 years.

Check content:

(1) Newborn visits: After the newborn is discharged from the hospital, doctors from the hospital or village doctors will visit the newborn at home, and the newborn will be visited 3-4 times within 28 days. The visit includes checking the general condition of the newborn, birth and vaccination status, comprehensive physical examination, breastfeeding guidance, etc. Any abnormal situation will be dealt with in a timely manner.

(2) Neonatal congenital disease screening When a newborn is born in the hospital, the doctor will collect blood from the heel of the newborn and make a filter paper blood smear for neonatal congenital disease screening. The project currently being carried out in our province is the screening of two congenital metabolic diseases: hypothyroidism and phenylketonuria.

(3) Regular physical examinations The child health doctor of the medical institution shall agree on the time for the child's regular physical examination according to the child's age. The examination contents shall include height, weight, hemoglobin, mouth, eyes, hearing, intelligence and blood tests. Parents shall take their children to the hospital under their jurisdiction for physical examinations at the time agreed by the doctor.

(4) Regular vaccinations When a child is born or moves to the local area, he/she should promptly go to the local hospital or township health center to apply for a "Child Vaccination Certificate". Each time a child is vaccinated, he/she should bring the "Child Vaccination Certificate" to the local hospital and follow the child immunization procedure.

(5) Prevention and treatment of common and frequently occurring diseases: Early screening, timely treatment and correction of nutritional diseases, infectious diseases, developmental diseases, as well as common and frequently occurring diseases in children such as amblyopia, strabismus, dental caries, and hearing loss.

(6) Specialist clinics: Medical and health care institutions at all levels have set up various pediatric specialist clinics, such as nutrition counseling, genetic eugenics, mental health, cerebral palsy rehabilitation, orthodontics, and amblyopia correction.

How to reissue the Child Health Handbook

The Child Health Handbook is a record of the child's growth and development. It is very important. Parents are requested to keep it properly. If you lose it by mistake, take your household registration booklet and vaccination certificate to the street prevention and treatment station (vaccination office) in your jurisdiction. The staff of the prevention and treatment station will give you the Liaoning Province Child Health Handbook for free. After that, you can take a physical examination for admission according to the requirements of the health care department. If the physical examination is normal, you can enter the kindergarten.
